The general data protection regulation (GDPR) comes into effect on 25th May 2018.
We take the protection of patients’ data very seriously and are therefore contacting you to ensure you are happy about the information we hold about you and how we contact you.
We hold the following information on our database/hard copy:
- Title, first name, surname
- Address and postcode
- Email address
- Mobile and landline numbers
- Credit / debit card details (when applicable)
- Medical history
The retention period for special data in patients’ records is a minimum of 10 years and may be longer in complex records in order to meet our legal requirements.
We only process personal data for the purposes of providing optimum healthcare.
Your personal information will never be passed to third parties unless we are making a professional referral for you. If we have your consent for referral to another health care provider we will send them just the information that we need to provide the necessary assessment, tests or treatments in a secure manner.
If we do not hear from you by 25th May, we will presume you are happy for us to hold the above information and to continue to receive correspondence from us by telephone, email or post.
